Oshi Health provides ongoing, personalized support to enhance the care you might already be receiving from a local GI clinic. We can provide support between visits and offer additional GI-specialized care to help you identify your triggers and make progress towards relief. And to ensure your care experience is coordinated, we can share records with your existing GI clinician at your request.

The most secure way to share your medical records is to send them to your GI Care Coordinator via chat in the Oshi Health mobile app. You can send screenshots, attachments, and links to our team by selecting a message with your GI Care Coordinator, and clicking the “+” sign at the bottom of Chat in the mobile app. You may also elect to email your documents to [email protected].

For most Oshi Health members, care is delivered through a fully virtual experience. You will meet with your GI providers through the Oshi Health mobile app to develop and make progress toward your individual care plan. There are some instances where in-person care – such as labs, imaging, or other diagnostic tests – is necessary.
If our care team refers you for external services (such as lab tests, in-person physical exams, or medications from your pharmacy), you will be responsible for costs. If you have insurance coverage, your standard copays and deductibles may apply. Oshi Health’s care coordination team strives to find in-network, affordable partners for external services.

Your Oshi Health GI provider can prescribe medications if needed, sending orders to the pharmacy of your choice. While Oshi’s care team strives to find affordable medication options (such as generics when possible), you will be responsible for medication costs. If you have insurance coverage, your standard copays and deductibles may apply.
For questions about refills on medication originally prescribed by an Oshi Health GI provider, please message your GI Care Coordination team via the Oshi Health mobile app or contact your pharmacy.

Having access to relevant medical records in advance of your appointment helps your GI provider prepare. Additionally, it gives your GI provider a better understanding of your medical history, which – paired with the information you share during the visit – helps them to make the most appropriate diagnostic assessment and care recommendations. It also reduces the likelihood of asking you to repeat tests.
- When completing your new patient intake form, please list prior procedures and/or surgeries (e.g.colonoscopies or endoscopies). Please include the provider name, date performed, and name and address where the procedure was performed.
- When completing your release of information form, please list current and/or prior GI doctors. Please include provider name, practice name and address, fax number, and phone number.
This information helps us request relevant medical records. Through the ROI form, you can also opt to have Oshi Health share recommendations and treatment plans back with your local provider(s) following your visit.
