    Medical services and medical records

    • While we are not an urgent care clinic, we offer the ability to get seen quickly, making us a great option for time-sensitive needs. If you are experiencing new or worsening symptoms, you can always reach out to Oshi Health at (646) 876-8455. By navigating the options, you can connect with a medical provider who can provide advice and help guide your next step, which could include helping you determine if an expedited appointment might be needed. As always, if you feel you need emergency care, please dial 911 or your local emergency number immediately.

    • The most secure way to share your medical records is to send them to your care coordinator via chat in the Oshi Health mobile app. You can send screenshots, attachments, and links to our team by starting a message with your care coordinator and clicking the “+” sign at the bottom of Chat in the mobile app. You may also email your documents to [email protected].

    • We follow the same high standard of care as any other medical practice. Your provider will take a thorough medical history, ask about your symptoms and treatment goals, and recommend blood tests or other labs if needed. Our care coordinators can help you with scheduling an in-person lab appointment at a convenient, in-network location. Please note that standard copays and deductibles may apply for any external services, as noted in your plan benefit details.

    • Your provider will review any prior testing to make sure we don’t order duplicate tests. They’ll recommend in-person diagnostics only when medically necessary.

      If your provider determines a need for in-person imaging (e.g., CT scan or MRI), a colonoscopy, or any other diagnostic procedure, we’ll work with you to sort out the logistics and ensure the location is in-network with your insurance.

      Please note that standard copays and deductibles may apply for any external services, as noted in your plan benefit details.

    • You can view your lab orders directly in the Oshi app. 

      Select “To-Dos” → “View Instructions” → “View Order.” 

      From there, you can print, email, or download a copy of your order.

    • Just like in traditional care settings, our Oshi Health GI provider can prescribe medications based on your care needs, sending orders to the pharmacy of your choice.

    • Having access to relevant medical records before your appointment helps your GI provider prepare. Additionally, it gives your GI provider a better understanding of your medical history, which, when paired with the information you share during the visit, helps them make the most appropriate diagnostic assessment and care recommendations. It also reduces the likelihood of asking you to repeat tests. 

      • When completing your new patient intake form, please list prior procedures and/or surgeries (e.g., colonoscopies or endoscopies). Please include the provider name, date of service, and the name and address of the facility where the procedure was performed.
      • When completing your release of information form, please list current and/or prior GI doctors. Please include the provider name, practice name and address, fax number, and phone number.

      This information helps us request relevant medical records. Through the ROI form, you can also opt to have Oshi Health share recommendations and treatment plans back with your local provider(s) following your visit.

    • Please email your care coordinator at [email protected] if you would like to get access to your medical records.

    • We are not able to see patients for the sole purpose of completing these forms. In some cases, we may be able to assist with FMLA or accommodations after you’ve established care with Oshi and our providers have completed a thorough evaluation to ensure it’s clinically appropriate. Our focus is on delivering quality care and resolving your symptoms. Please note that these requests are considered only for gastrointestinal conditions after the first visit and are not guaranteed.
